Job Summary
Implement the nursing process through assessment, diagnosis, planning, implementation, and evaluation for adult and pediatric patients in their own homes. Provide specialized care including tracheostomy management, ventilator support, feeding tube administration, wound care, and seizure management. Communicate regularly with physicians and the multidisciplinary patient care team regarding patient health status. Maintain accurate documentation and adhere to core values of accountability, compassion, integrity, and teamwork. This role supports faster recovery for individuals at home, requiring a valid Minnesota LPN license, BLS CPR certification, and the ability to lift up to 50 lbs.
Required Qualifications
- Valid license as a Licensed Practical Nurse (LPN) in the state of Minnesota
- Ability to read, write and speak fluently in English
- Education and experience in the scope of services offered by the agency
- Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies
- Must be able to lift, turn, position, and transfer up to 50lbs
- Drug free work environment
