Job Summary
Assess, diagnose, plan, implement, and evaluate patient care for adults and children in their own homes. Manage complex needs including tracheostomy, ventilators, feeding tubes, wound care, and seizure management. Communicate health status updates to physicians and the multidisciplinary care team. Maintain accurate documentation as per our standard operating principles. Requires a valid Minnesota LPN license, BLS CPR certification, and ability to lift up to 50 lbs. Typical shifts are 8-12 hours.
Required Qualifications
- Valid license as a Licensed Practical Nurse (LPN) in the state of Minnesota
- Ability to read, write and speak fluently in English
- Education and experience in the scope of services offered by the agency
- Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies
- Must be able to lift, turn, position, and transfer up to 50lbs
- Drug free work environment
