Job Summary
Implement the nursing process through assessment, diagnosis, planning, implementation, and evaluation while communicating with physicians and multidisciplinary teams regarding patient health status. Provide one-on-one adult and pediatric nursing care in the patient's home, managing complex needs such as tracheostomy, ventilators, feeding tubes, wound care, and seizure management. Maintain accurate documentation and adhere to strict operational principles where if not documented, it did not happen. Typical shifts are 8-12 hours. Valid Minnesota LPN license, BLS CPR certification, and ability to lift 50 lbs required.
Required Qualifications
- Valid license as a Licensed Practical Nurse (LPN) in the state of Minnesota
- Ability to read, write and speak fluently in English
- Education and experience in the scope of services offered by the agency
- Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies
- Must be able to lift, turn, position, and transfer up to 50lbs
- Drug free work environment
