Job Summary
Provide one-on-one nursing care in the patient’s home as a Private Duty LPN, performing the nursing process (assessment, diagnosis, planning, implementation, evaluation) and communicating with physicians and the multidisciplinary care team. Care may include tracheostomy and ventilator support, feeding tubes, wound care, and seizure management. Role requires a valid Minnesota LPN license, English proficiency, ability to lift/position/transfer up to 50 lbs, and adherence to the agency’s core values. Shifts typically 8-12 hours with competitive hourly pay and benefits (health insurance reimbursement, 401k, CEUs, PTO, on-demand pay) and a referral bonus; location Hawley, MN.
Required Qualifications
- Valid Licensed Practical Nurse (LPN) license in Minnesota
- Basic Life Support (BLS) CPR certification through American Heart Association or Red Cross (renewed as required)
