Job Summary
Provide one-on-one nursing care in the patient’s home for adult and pediatric clients as a Licensed Practical Nurse (LPN). Implement the nursing process (assessment, diagnosis, planning, implementation, evaluation) and communicate with the physician and multidisciplinary care team regarding patient health status. Deliver a range of cares from tracheostomy and ventilator care to feeding tubes, wound care, and seizure management. Must hold a Minnesota LPN license, be fluent in English, and be able to lift/transfer up to 50 lbs. Typical shifts are 8–12 hours. Required Qualifications
- Licensed Practical Nurse (LPN) license in Minnesota
- Basic Life Support (BLS) CPR certification
- English fluency (read, write, speak)

- Ability to lift, turn, position, and transfer up to 50 lbs
