Job Summary
Provide adult and pediatric nursing care in patients' homes on a one-to-one basis. Implement the nursing process (assessment, diagnosis, planning, implementation, evaluation) and communicate with the physician and multidisciplinary care team regarding patient health. Deliver diverse cares from tracheostomy and ventilator support to feeding tubes, wound care, and seizure management. LPN license in Minnesota and English fluency required; BLS CPR certification required; ability to lift up to 50 lbs. 8-12 hour shifts with benefits including health insurance reimbursement, 401k, CEU opportunities, PTO, and on-demand pay.
Required Qualifications
- Valid LPN license in Minnesota
- Ability to read, write and speak English
- Basic Life Support (BLS) CPR for healthcare providers (AHA or Red Cross)
- Must be able to lift, turn, position, and transfer up to 50 lbs
- Education and experience in the scope of services offered by the agency
