Job Summary
Licensed Practical Nurse (LPN) to provide prescribed medical treatment and personal care to patients at the Community Based Outpatient Clinic in Danville. Collaborates with the PACT Team and extended care partners; performs nursing duties including patient care, routine and annual nursing assessments, administering medications, operating diagnostic equipment (EKG, glucometers, spirometry, oxygen equipment, etc.), collecting lab specimens, documenting in VISTA/CPRS, and assisting with front desk duties. Responsible for patient and family health education, maintaining patient confidentiality, and complying with VA and clinic training requirements to achieve excellent clinical outcomes and customer service for veterans.
Required Qualifications
- Diploma or certificate of an accredited practical or vocational nursing program (approved by the state)
- Minimum one-year experience as an LPN/LVN in a related primary care or ambulatory care setting (government healthcare environment preferred)
- Must be credentialed and remain in good standing through the Veterans Health Administration (VA)
- Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements
- Current Basic Life Support (BLS) certification and ACLS as specified by VA contract
- Proficiency in written and spoken English
- Strong computer skills, including EMR experience
