Licensed Practical Nurse - Physician Practice

Provides nursing care as part of a care team to improve quality and patient experience in a physician-practice setting. Responsibilities include assisting the provider by collecting patient history and reason for visit, performing observation and physical exams, conducting telephone triage and analysis of diagnostic data, ensuring laboratory and diagnostic tests are tracked and that results are available to the provider, delivering patient education, assisting with prescriptions, scheduling and referral coordination, and assisting during procedures and examinations. Coordinates internal and external appointments based on patient medical needs and practice schedule, intervenes to prevent or minimize risks, participates in telephone triage per established protocols, and supports overall care coordination and patient safety in a day-shift, onsite environment.
