Job Summary
Per Diem LPN providing hands-on patient care across hospitals, skilled nursing facilities, rehabilitation centers, nursing homes, and assisted living communities. Deliver monitored vital signs, administer medications, wound care, injections, blood draws, catheter care, and assist with admissions, discharges, and transfers. Collaborate with RNs and interdisciplinary teams; maintain accurate documentation; participate in team huddles; ensure effective communication with patients and families; manage supplies and restock as needed.
Required Qualifications
- Graduate of an accredited Licensed Practical Nurse (LPN) program
- Active, unrestricted LPN license in the state of practice
- Minimum 6 months of recent LPN experience (1–2 years preferred)
- Current CPR/BLS certification
- Ability to push, pull, support, and lift a minimum of 80 pounds
- Physical stamina to manage long hours of standing and walking
- Annual physical examination (where required)
- Negative TB test results
- Clean background check
- COVID-19 vaccination and boosters, as required by CMS or state regulations
