Job Summary
Conduct health screenings, manage medication regimens, and provide direct treatment for clients with severe mental illness in home and community settings. Assess physical health needs, monitor vital signs, and make referrals to community physicians while developing nursing care plans. Deliver crisis intervention and emergency care as needed, ensuring at least 80% of monthly treatment contacts occur in the community. Complete required documentation including progress notes and incident reports per New York State regulations. Participate in 24/7 on-call rotation and be available for flexible hours, weekends, and holidays.
Required Qualifications
- Licensed Practical Nurse (LPN) with current New York State license
- Associates Degree in Nursing or equivalent
- Knowledge of professional nursing concepts, principles and practices
- Knowledge of abnormal signs and symptoms relating to physical and mental illness, as well as indicators of wellness
- Excellent oral, writing and communication skills
- Ability to work flexible hours and days – including weekends/evenings/holidays according to needs of a 24/7 program
- Regularly required to talk, hear, walk, stand & sit
- Able to lift up to 10 pounds
- Able to climb stairs and make home visits
- Able to travel to multiple locations as needed
- Participate in 24/7 on-call rotation and crisis response
Desired Qualifications
- Bachelor's Degree Preferred
- One-year post LPN licensure clinical nursing experience working with individuals with behavioral health needs preferred
- Experience providing community-based treatment to individuals living with severe mental illness preferred
- Bilingual Spanish speaking, preferred
