Licensed Practical Nurse

Implement the interdisciplinary resident care plan by providing direct care and overseeing CNA team members. Review each resident's status daily, identify and report condition changes, and ensure resident choices regarding care are honored. Lead the CNA team in following comprehensive care plans, delegate tasks based on licensure and expertise, and supervise delivery to meet facility policies. Participate in care delivery, medications, and treatments as assigned, while evaluating outcomes and adjusting care plans accordingly. Counsel employees with performance discrepancies and document all care and reports as required.
