Licensed Practical Nurse Outpatient PACE Program

LPN in the Outpatient Clinic PACE program at CarePartners Health Services provides direct patient care in an outpatient setting, overseeing medications, coordinating with the interdisciplinary team, and supporting care continuity to promote independence for participants in their homes and communities.
