Job Summary
LPN in the Outpatient Clinic PACE program at CarePartners Health Services provides direct patient care in an outpatient setting, overseeing medications, coordinating with the interdisciplinary team, and supporting care continuity to promote independence for participants in their homes and communities. Responsibilities include evaluating patient vitals, assisting with procedures, administering oral/injectable medications, triaging participant needs, coordinating with specialty providers and nursing facilities, and contributing to care plans and documentation to ensure optimal outcomes for elderly participants in the PACE program. The role emphasizes interdisciplinary collaboration, patient advocacy, and adherence to legal, ethical, and clinical standards within an outpatient clinic schedule (Monday–Friday; no weekend shifts).
Required Qualifications
- Associate Degree in Nursing (ADN) or equivalent
- Licensed Practical Nurse (LPN/LVN) license
- Basic Cardiac Life Support (BCLS) certification within 30 days of employment start date
- LPN/LVN licensure validation
- Clinical experience in outpatient or similar setting preferred
- Ability to delegate to licensed/unlicensed staff according to scope of practice
- Proficiency in documentation and patient education
