Licensed Practical Nurse Outpatient PACE Program

Provide direct patient care in an outpatient setting by evaluating outcomes, recording vitals, and administering oral or injectable medications while monitoring patient responses. Coordinate care with specialist providers, nursing facilities, and the interdisciplinary team to implement goals and interventions, triaging participants to determine if physician visits are necessary. Document patient progress, individualize teaching and care plans, and delegate tasks to licensed and unlicensed staff according to scope of practice. Work Monday through Friday in the PACE program, supported by a team dedicated to promoting independence and community-based living for elderly participants.
