Job Summary
Licensed Practical Nurse (LPN) within the PACE program provides direct patient care in an outpatient clinic, administers oral and injectable medications, performs vitals assessment, triage, and coordinates care with an interdisciplinary team to support participants in the community. Role emphasizes medication oversight, care coordination, and documentation, with Monday-Friday schedule and no travel. Requires an Associate Degree and Basic Cardiac Life Support within 30 days of employment; LPN/LVN licensure is required.
Required Qualifications
- Licensed Practical Nurse (LPN/LVN) license
- Associate Degree in Nursing (or equivalent)
- Basic Cardiac Life Support (BCLS) must be obtained within 30 days of start date
- No Travel Required
- Demonstrated ability to provide direct patient care in outpatient settings
- Ability to administer oral and injectable medications as ordered
- Ability to coordinate care with interdisciplinary team and care facilities
- Strong communication and documentation skills
