Job Summary
LPN in outpatient PACE program providing direct patient care and medication oversight in an outpatient clinic. Responsibilities include evaluating vitals, administering medications, coordinating care with the interdisciplinary team and care facilities, triaging participants, and supporting care plans to promote independence and community living. Requires an Associate Degree and current LPN/LVN license; BCLS within 30 days of start; licensure within 2 years. Monday–Friday schedule with no travel; comprehensive benefits package available.
Required Qualifications
- Associate Degree (LPN/LVN)
- Licensed Practical Nurse license
- Basic Cardiac Life Support (within 30 days of start)
- LPN/LVN licensure obtained within 2 years of employment start date
