Job Summary
Licensed Practical Nurse (LPN) in an outpatient PACE program provides direct patient care and medication oversight within an interdisciplinary team. Responsibilities include administering medications (oral/injectable), documenting patient responses, evaluating vitals, coordinating with providers, care team, and SNF partners, triaging participant needs, and developing/implementing care plans to promote independence and home/community-based living. Positioned Monday–Friday in an outpatient clinic with no travel required; collaborates with caregivers, pharmacy, and the interdisciplinary team to ensure safe, high-quality care in a geriatric, outpatient setting.
Required Qualifications
- Associate Degree in Nursing (LPN/LVN)
- Basic Cardiac Life Support (BLS) certification obtained within 30 days of employment
- LPN/LVN licensure (within 2 years of employment)
