Licensed Practical Nurse Outpatient PACE Program

LPN in outpatient clinic within the PACE program providing direct patient care, medication administration and documentation, coordinating with an interdisciplinary team to support participants' independence and community-based living. Responsibilities include evaluating vitals, assisting with procedures, administering medications, delegating within scope, triaging participant needs, coordinating with specialists and nursing facilities, and developing/implementing care plans to promote optimal outcomes. Requires an Associate Degree, BLS within 30 days of start, and LPN/LVN licensure. Works Monday–Friday with no weekend shifts; no travel.
