Job Summary
LPN responsible for direct patient care in an outpatient PACE clinic, including medication oversight and documentation, triage, care coordination, and collaboration with providers, pharmacy, and the interdisciplinary team to promote independence and safe community living for participants.
Required Qualifications
- LPN/LVN license
- Associate Degree in Nursing
- Basic Life Support (BLS) certification obtainable within 30 days of employment
- ability to administer oral/injectable medications
- clinical nursing experience in outpatient setting
- ability to delegate within scope of practice
- strong documentation and patient education skills
