Job Summary
Conduct nurse visits in clinics performing INR checks, coumadin management, vaccine administration, medication administration, heart monitor placement, EKG, and sleep device instruction. Manage triage calls from patients and families regarding medications, prescriptions, referrals, immunizations, lab results, and illness symptoms. Process complex multidisciplinary messages from providers, notifying patients of abnormal laboratory and imaging results requiring further workup. Serve as liaison between providers, care management teams, and patients regarding quality-of-care initiatives via daily huddles, face-to-face, and telephone communication. Participate in health education pertinent to preventive care. This full-time role includes rotating Saturdays in the Londonderry clinic and offers paid time off, comprehensive insurance, and 401K.
Required Qualifications
- Graduation from an accredited school of nursing
- Current State of New Hampshire Nurse/LPN license
- Ability to communicate effectively, both orally and in writing
- Minimum of two years of acute care or ambulatory care experience
- Physical presence in the Windham, NH office 4 days/36 hours per week
- Availability for rotating Saturdays (every 12 weeks) in the Londonderry clinic
