Job Summary
Licensed Practical Nurse (LPN) in Home Health providing in-home skilled nursing care per plan of care. Deliver high-quality clinical services within scope of practice and infection control procedures, document visits thoroughly in the EMR, and participate in care coordination with patients, families, physicians, and the health care team. Communicate changes in patient condition, report needs, attend case conferences, and adhere to regulatory and payer requirements. Required Qualifications
- Current and unrestricted LPN licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Ability to work flexible hours as required to meet identified client needs
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
