Job Summary
Provide nursing care to patients and significant others via direct and/or telephone contact, following established standards. Coordinate with the care team to ensure seamless delivery and active patient participation in planning. Assist physicians with clinical procedures and participate in patient teaching. Direct clerical assistants, medical assistants, and other non-licensed personnel. Work Monday through Friday between 7:30am and 5:30pm. This role supports a Patient Centered Medical Home model across New York, New Jersey, and Southern Connecticut, focusing on preventative medicine through treatment.
Required Qualifications
- Graduate of an accredited school of Practical Nursing
- New York State (NYS) license with current NYS registration, in good standing
Desired Qualifications
- Current CPR/BLS certification
- 1+ years of experience as a licensed practical nurse
- 2+ years as a Medical Assistant in a pertinent practice area
