Job Summary
Monitor health conditions and administer medications while overseeing staff compliance and providing prescribed treatments per physician orders. Conduct assessments to implement Individual Service Plans and respond to condition changes. Provide training to staff on universal precautions and infection control, and educate individuals on self-medication, STD prevention, and smoking cessation. Serve as a liaison with medical providers and advocate for individuals served. Requires current state LPN license, one year of experience, and CPR/FA and HIV/AIDS certifications. Full, part-time, and as-needed schedules available with comprehensive benefits.
Required Qualifications
- Associate's Degree in Nursing or related field, or a graduate of an accredited LPN program
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN License
- Current driver's license, car registration, and auto insurance
- Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
- Ability to effectively communicate with staff, individuals served, and families
