Job Summary
Monitor health conditions of individuals served and ensure quality services are provided. Administer medications, oversee staff administering drugs, and provide prescribed medical treatments per physician orders. Conduct assessments, implement Individual Service Plans (ISP), and respond to changes in condition. Provide training to staff on universal precautions, infection control, and medication administration, while also educating individuals on self-medication, STD prevention, and smoking cessation. Serve as a liaison and advocate with medical and professional service providers. Requires current state LPN license, one year of experience with the population served, and ability to communicate effectively with staff, individuals, and families.
Required Qualifications
- Associate's Degree in Nursing or related field, or a graduate of an accredited LPN program
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN License
- Current driver's license, car registration, and auto insurance
- Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
- Ability to effectively communicate with staff, individuals served, and families
