Job Summary
Monitor health conditions and administer medications while overseeing staff who do so. Provide prescribed medical treatment and conduct assessments to implement Individual Service Plans for individuals with intellectual disabilities, brain injuries, or substance use disorders. Train staff on universal precautions and infection control, and educate individuals on self-medication, STD prevention, and smoking cessation. Serve as a liaison with external agencies to coordinate consistent treatment and advocate for individuals receiving care. This role offers high-impact work within a mission-driven team at Mentor Community Services, a Sevita family provider of community-based services for people with developmental disabilities.
Required Qualifications
- Associate's Degree in Nursing or related field, or state certificate
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure for state of residence
- Current driver's license, car registration, and auto insurance
- Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
- Ability to effectively communicate with staff, individuals served, and families
