Job Summary
Monitor health conditions and administer medications while overseeing staff documentation and treatment under physician orders. Conduct assessments to implement Individual Service Plans, respond to condition changes, and provide prescribed medical and personal care services. Train staff on universal precautions, infection control, and medication administration, while educating individuals on self-medication, STD prevention, and smoking cessation. Serve as a liaison with medical providers and advocate for individuals with intellectual and developmental disabilities. Requires current state LPN license, one year of relevant experience, and ability to communicate effectively with families and staff.
Required Qualifications
- Associate's Degree in Nursing or related field, or a graduate of an accredited LPN program
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN License
- Current driver's license, car registration, and auto insurance
- Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
- Ability to effectively communicate with staff, individuals served, and families
