Job Summary
Monitor health conditions and administer medications while overseeing staff who provide prescribed medical treatments and personal care services. Conduct assessments to implement health care services within each Individual Service Plan, responding promptly to changes in individual conditions. Provide training to staff on universal precautions, infection control, and medication administration, while also educating individuals served on self-medication, STD prevention, and smoking cessation. Serve as a liaison with medical providers and external agencies to coordinate consistent treatment. This role supports individuals with intellectual and developmental disabilities, brain injuries, and substance use disorders within Sevita's community-based care network.
Required Qualifications
- Associate's Degree in Nursing or related field, or state certificate
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure for state of residence
- Current driver's license, car registration, and auto insurance
- Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
- Ability to effectively communicate with staff, individuals served, and families
