Job Summary
Deliver direct patient care including medication administration, wound care, and procedures under registered nurse supervision. Collaborate with interdisciplinary teams comprising aides, social workers, and chaplains to address patient and family needs. Monitor and document changes in patient conditions while communicating effectively to the care team. Work Monday through Friday from 8 AM to 5 PM with an eighth-weekend rotation. This role supports Mountain Valley Hospice's mission to provide personalized end-of-life care in homes, skilled nursing facilities, and assisted living settings.
Required Qualifications
- Valid Licensed Practical Nurse License
- Exceptional interpersonal and communication skills
- Compassionate and patient-centered approach
- Ability to work independently
- Ability to work as part of a team
- Excellent time management skills
- Must be available for Monday - Friday schedule
- Must be available for 8-hour days
- Must be available for 8 AM to 5 PM shift
- Must be available for weekend rotation every 8th weekend
