Licensed Practical Nurse - LTC - Rehab - Day Shift

Provide direct patient and resident care, administer medications and treatments, and perform nursing assessments to monitor conditions. Develop and implement individualized care plans while collaborating with physicians, therapists, and interdisciplinary teams. Support and supervise nursing assistants, assist with admissions and discharges, and coordinate rehabilitation care. Document patient care accurately within the medical record to maintain a safe, compassionate, and patient-centered environment. This role operates in Levindale Hospital's Hall 1 Rehab/Subacute Care unit, a fast-paced subacute rehab environment caring for patients with post-surgical recovery, orthopedic rehabilitation, cardiac conditions, and wound care needs.
