Job Summary
Implement and maintain Individual Service Plans while conducting ongoing physical and mental assessments for consumers. Administer prescribed medications, monitor compliance and side effects, and assess vital signs, weight, and laboratory tests as ordered. Maintain accurate consumer records, ensure safety, and supervise medical treatment programs across residential, crisis, and assisted living settings. Complete prior authorizations and patient assistance for psychiatric medication needs. Adhere to all agency policies and relevant legal requirements.
Required Qualifications
- Graduation from an accredited Licensed Practical Nurse Program
- Current Virginia unrestricted LPN License or Current Unrestricted LPN License from a Nurse Licensure Compact State
- 1-year LPN work experience in a mental health program and/or residential setting
- 1-Year LPN work experience involving individuals with intellectual disabilities
- Valid driver's license with acceptable DMV record
- Proficiency in Microsoft Office
Desired Qualifications
- Mental Health and/or ID/DD experience is preferred for this position
- Experience using Electronic medical records
- basic knowledge of Medicaid/Medicare Regulations
