Licensed Practical Nurse (LPN) - Wound Care

Collect and document patient data, observations, and responses to care per TriHealth guidelines. Communicate assessment findings and patient needs to the RN and interdisciplinary team while contributing to the development, implementation, and updates of the patient plan of care. Provide direct basic nursing care, administer medications within scope, and perform technical skills using equipment safely. Prioritize workload, recognize changes in patient condition, and initiate emergency measures as needed. Maintain effective interdisciplinary relationships and support process improvement to foster professional growth.
