Job Summary
Provide in-home Licensed Practical Nurse (LPN) care to pediatric and adult patients, performing head-to-toe assessments and following individualized plans of care. Care may include G-tube feedings, medication administration, wound care, trach care, and ventilator support. Educate patients and caregivers on disease management, emergency procedures, and therapeutic techniques. Collaborate with physicians, therapists, and other team members to optimize care. Document all care activities accurately and promptly in compliance with agency and state requirements. Maintain rigorous safety, infection control, and privacy standards. Shift options range from 4 to 12 hours with flexible daytime, evening, and weekend scheduling; opportunities for consistent long-term or variable schedules.
Required Qualifications
- Active LPN license in Indiana or an active compact LPN license
- At least 1–2 years of clinical nursing experience (home health and pediatric experience strongly preferred)
- Experience with G-tube, trach, or vent care preferred (but training will be offered)
- Current CPR certification
- Strong critical thinking and independent decision-making abilities
- Excellent interpersonal and communication skills
- Reliable transportation and a valid driver’s license
