Job Summary
Manage patient care at home by conducting 16-18 visits per weekend, ensuring coverage of high-priority cases. Case manage, assess, plan, and implement comprehensive home health care plans while measuring vital signs, administering IVs, changing dressings, and cleaning wounds. Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments across various patient populations. Collaborate with physicians, therapy, social services, and home health aides to support your caseload, and deliver patient and family education for disease management and health promotion. Work weekends from 7:00 AM to 5:30 PM.
Required Qualifications
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification
- Weekend Day shift: 7:00AM to 5:30PM
- Provide 16-18 visits per weekend
