Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. Assess and document patient conditions, administer medication and IV fluids, and inspect for injuries or bedsores to deliver appropriate treatments. Change dressings, assist with personal hygiene, and collaborate with providers and allied health professionals to ensure exceptional patient outcomes. Schedule includes weekend hours in Allegheny and Westmoreland County areas with flexible full-time, part-time, or PRN options. No overtime required.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as a LPN/LVN in the state(s) in which position is located
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
Desired Qualifications
- Practical trach and/or ventilator experience
