Job Summary
Licensed Practical Nurse (LPN) for home health in Wabash, providing individualized care to pediatric and adult patients in their homes. Responsibilities include head-to-toe assessments, G-tube feedings, tracheostomy and ventilator support, medication administration, wound care, and care for patients with trach/vent needs. Educate patients and caregivers on disease management and emergency procedures; collaborate with physicians, therapists, and other team members; document all care activities in compliance with agency/state requirements; maintain safety, infection control, and privacy standards. Shift options range from 4 to 12 hours with flexible daytime/evening/weekend scheduling and opportunities for long-term cases.
Required Qualifications
- Active LPN license in Indiana or active compact LPN license
- Current CPR certification
- At least 1–2 years of clinical nursing experience (home health and pediatric experience strongly preferred)
