Licensed Practical Nurse (LPN)

Provide individualized, goal-directed nursing care under RN supervision by implementing physician orders and carrying out patient care plans. Ensure timely documentation of visits, communicate activities to the RN, and evaluate care effectiveness to meet patient needs. Perform patient and caregiver education on medications, treatments, and discharge instructions while identifying interdisciplinary requirements. Coordinate care with physicians and team members, transmit incident reports to the QI Specialist, and advocate for sensitive, high-quality patient outcomes. Requires a valid LPN license, acute care experience, and flexibility for rotational weekends and holidays.
