Licensed Practical Nurse (LPN) Variable Shifts

Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. Assess and document patient conditions including vitals, blood, stool, urine, and glucose levels, then report changes as needed. Administer medication, insulin, and IV/fluids with thorough documentation while inspecting and caring for wounds, changing dressings, and assisting with personal hygiene. Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes. This role covers two different home care cases in Harrison City across days, nights, school, and non-school hours.
