Licensed Practical Nurse (LPN) at Valencia Hills Center for Rehabilitation & Healing in Lakeland, FL. Responsibilities include Admissions, Discharge and Transfers of residents; Medication requisitions with pharmacy and physicians; Medication administration; Following infection control, emergencies, and other center policies; Collaborating with resident’s interdisciplinary team to provide the best outcomes; Identifying and reporting all abnormal changes in residents’ condition; Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment; Delivers nursing care to patients/residents requiring long-term or rehabilitative care; Collects patient/resident data, makes observations, and reports pertinent information; Implements and evaluates the patient/resident plan of care per state regulations; Directs and supervises care given by other nursing personnel in selected situations; Maintains knowledge of documentation requirements and equipment usage; Maintains patient confidentiality and rights.
