Job Summary
Conduct thorough patient assessments, administer prescribed medications, and provide assistance with Activities of Daily Living in home settings. Educate patients and families on health maintenance while coordinating schedules directly with patients to align with their approved plans of care. Monitor patient symptoms, report changes promptly, and maintain accurate documentation in the EMR system. Communicate seamlessly with case managers and physicians to ensure a unified approach to patient care. Offer emotional support and clear clinical notes reflecting each patient's journey.
Required Qualifications
- A valid and active LPN License in the state of practice.
- A willingness to travel, bringing care directly to patients' homes.
- Graduation from an accredited school of nursing.
- Current BLS Certification.
- Ability to pass a state & federal background check, ensuring trust and safety.
- Malpractice Insurance is required.
- Strong problem-solving and critical thinking skills, coupled with a warm, empathetic approach.
