Licensed Practical Nurse (LPN)

Transitions At Home is hiring a full-time Licensed Practical/Vocational Nurse (LPN) to provide skilled nursing care to patients in their homes in the Janesville, WI area. Responsibilities include routine visits, medication management, treatments, coordinating services, and counseling the patient and family/significant others in accordance with physician orders and plan of care under the supervision of the RN Clinical Manager. Hours are 40 per week, Monday–Friday, with no weekends and limited on-call rotation.
