Job Summary
Provide skilled nursing care in patients' homes, including routine visits, medication management, treatments, and coordination of services under the direction and supervision of the RN Clinical Manager, following the physician orders and plan of care. Role is full-time, 40 hours/week, Monday–Friday 8:00 am–4:30 pm with no weekends and limited on-call rotation. Required Qualifications
- Active Wisconsin LPN License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
