Job Summary
Licensed Practical Nurse (LPN) provides direct patient care under the direction of the RN in the ElderONE Transitional Housing setting. Responsibilities include observing patient medical status on admission and throughout treatment, assisting in implementing the treatment plan under RN direction, delivering routine nursing services as directed by the RN, completing accurate documentation, providing regular patient status updates, participating in department/facility meetings, maintaining clinical competency through continuing education, and performing other duties as assigned. Requires BLS certification, NY LPN license, AS in Nursing, and 1 year experience preferred; shifts include 7:00 AM–2:00 PM, 5:00 PM–9:00 PM, with additional early morning/evening hours to meet a 24-hour schedule; location at ElderONE campuses in Rochester, NY and Hudson, NY.
Required Qualifications
- Graduate from an accredited School of Practical Nursing
- Current LPN licensure in State of New York
- Basic Life Support (BLS) Certification from the American Heart Association (AHA)
