Job Summary

Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring. Observe, assess, and report changes in resident condition to physicians and the nursing team. Assist in developing and modifying resident care plans, while documenting care accurately in electronic health records. Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care, and participate in facility surveys and quality improvement initiatives. Maintain a safe and therapeutic environment for residents in a fast-paced setting with frequent interruptions. May require working beyond normal hours, including weekends and holidays.

Required Qualifications

  • Current, unencumbered LPN license
  • Ability to make independent decisions and exercise sound judgment
  • Strong communication and interpersonal skills, with the ability to interact tactfully with residents, families, and staff
  • Knowledge of nursing and medical practices, procedures, and long-term care regulations
  • Ability to work effectively in a team environment and supervise other personnel as needed
  • Patience, compassion, and a positive attitude
  • Ability to lift, push, pull, and transport residents with assistance as needed
  • May require working beyond normal hours, including weekends and holidays
