Job Summary
Provide direct patient care as an LPN in a home-health setting, document care per the plan of care, assess and educate family/caregivers, and assist the RN or physician with specialized procedures. Coordinate home health services with other disciplines, observe aseptic technique, and assist patients in learning self-care. Care for patients with trachs and vents and ensure safety and comfort in patients' homes. Requires LPN license, CPR/PPD certifications, 1+ year experience, valid driver’s license, and reliable transportation.

- 1+ year experience as an LPN
- Valid LPN License
