Job Summary
Licensed Practical Nurse (LPN) position providing nursing and supportive care in the patient’s residence or facility under the direction of an RN, following the Interdisciplinary Plan of Care; coordinate with attending physicians and home health team to optimize patient care and community resource use; document services; demonstrate knowledge of Medicare/Medicaid regulations for home health benefits; report changes in patient condition to the RN; requires AR LPN license, 1 year clinical experience, CPR certification, and reliable transportation for patient visits. Required Qualifications
- Current Arkansas Licensed Practical Nurse (LPN) license required
