Licensed Practical Nurse (LPN)

Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care under the direction of the RN, communicating with physicians and team members to coordinate optimal patient care and community resources. Document all services provided and demonstrate knowledge of Medicare/Medicaid regulations for home health benefits. Report changes in patient condition and needs to the RN immediately. Requires a current Louisiana LPN license with at least two years of experience, dependable transportation, and a driver's license.
