Job Summary
Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN; communicate with attending physicians and home health team to coordinate optimal care and use of community resources for the patient/family; document all services provided; demonstrate knowledge of Medicare/Medicaid regulations for home health benefits; report changes in patient condition and needs to the RN; patient visits occur at the patient’s residence or facility; requires current Louisiana LPN license, minimum 1 year clinical experience, CPR certification, and reliable transportation; part-time position with ESOP participation and mileage reimbursement.
Required Qualifications
- Current Louisiana Licensed Practical Nurse (LPN) license required
- Minimum 1 year clinical experience as an LPN
- Current CPR Certification
- Reliable transportation to conduct patient visits at their place of residence or facility
