Job Summary
Part-Time Licensed Practical Nurse (LPN) needed for home health visits in Lafayette, LA. Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care under the direction of the RN; communicate with attending physicians and other team members to coordinate optimal patient care and use of community resources; document all services; demonstrate knowledge of Medicare/Medicaid regulations for home health benefits; report changes in patient condition to the RN.
Required Qualifications
- Current Louisiana Licensed Practical Nurse (LPN) license
- Minimum 1 year clinical experience as an LPN
- Current CPR Certification
- Reliable transportation to conduct patient visits at their place of residence or facility
