Licensed Practical Nurse (LPN)

Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care under RN direction. Communicate with attending physicians, team members, and agencies to coordinate patient care and community resources. Document all services provided and demonstrate knowledge of Medicare/Medicaid regulations. Report changes in patient condition and needs to the RN. Maintain current Florida LPN licensure, one year of clinical experience, and CPR certification. Conduct patient visits at residences or facilities using reliable transportation.
