Licensed Practical Nurse - LPN - St. Louis Rehab

Assist the RN with admission and discharge processes, perform patient assessments, and conduct handoffs using safety tools like SBAR. Collaborate with the team to evaluate patient progress, document interventions, and reinforce education for patients and families. Delegate tasks to UAPs, complete required certifications, and serve as a preceptor for students or new hires when needed. Perform additional responsibilities as assigned to support patient care and departmental goals within our multidisciplinary rehabilitation programs.
