Licensed Practical Nurse (LPN)- Skilled Nursing- Part Time

Part-time LPN at Frederick Living (Evenings 2:30pm-11:00pm or Nights 10:30pm-3:00am, every other weekend and holiday rotation) providing skilled nursing care to residents while leading the neighborhood team. Responsibilities include observing and reporting changes in resident condition, communicating with supervisors and care providers, applying evidence-based practice for safe, high-quality care, supervising staff through performance evaluation and coaching, completing accurate documentation, participating in care conferences and transitions, collaborating with Cedarwood staff, ensuring infection control, maintaining hand-held equipment, and attending required trainings and team meetings. Requires LPN license in Pennsylvania, high school diploma or equivalent, BLS certification, basic computer skills, strong communication skills, and a commitment to person-centered care.
